Daycare/Preschool Health Statement

Please call or send a portal message to request a daycare health statement. If your child has been seen in our office within the last year, our staff will complete the form and upload it to your portal. To access and print the document:

Log in to your patient portal
Click Documents
Select the document you would like to print
Click Print in the upper right-hand corner
